Directions for filling out the CHS Swim Team Form

1. Please complete all fields at the top of the form. Be sure to circle the following:
   1. Team – swim or dive
   2. Grade – 9, 10, 11, or 12
   3. JV or V (Varsity)
   4. High School – CHS or NTH@C (New Tech)

   II.   Booster Club Dues - $50

   1. NOTE: If you paid on 5/15 and didn’t receive your window decal, please let Jennifer Kuechler know.

   III.  Flamingo Flocks – this is required for our FUNdraiser in the fall

   1. Freshman: Full flock, $32 (unless you opt to pay $100 donation instead of participating in the FUNdraiser)
   2. Soph – Senior: If you need additional flamingos, you can purchase 5 more birds for $16

   IV. Volunteer Section – please check the box(es) you would be interested in helping out with during the season.

             WE NEED EVERYONE TO PITCH IN TO MAKE THE SEASON SUCCESSFUL!

       V.  Swim/Dive Fees Section

1. 1.  Swim Suit/T-Shirt – Required
      1. Girls - $72 includes:
         1.   Suit (Coach will help with sizing)   
         2.  T-shirt (circle the correct size – all adult sizes)
         3. Two (2) caps – one black and one white - latex
      2. Boys - $62 includes:
         1. Suit (Coach will help with sizing)    
         2. T-shirt (circle the correct size – all adult sizes)
         3. Two (2) caps – one black and one white - latex

       b.     Booster Club Membership - $50

               c.     EXTRA Caps – NOT REQUIRED! Note here if your swimmer needs a silicon  

                       cap due to allergies, etc., otherwise he/she will receive latex caps.

                             i.   ONLY to be filled out to buy EXTRA caps in addition to the

                                  two (2) you receive with the price of your suit and t-shirt.

               d.      Flamingo Flocks – Required (unless you opt to pay $100 donation instead of

                                                                         participating in the FUNdraiser)

       VI.   Please write your total and include check number, if applicable.

                a.     Payable to the CHS Swim & Dive Booster Club

                b.     Mail the form and check/cash to:   David Cain, Treasurer
